TPG Telecom has revealed a possible second fatal Triple Zero failure linked to a Samsung handset, telling a Senate inquiry it only learned this week that a September emergency call problem may also have involved a death, on top of last month’s fatal incident in Sydney.

The telco’s chief executive Inaki Berroeta used his opening statement at the Senate inquiry to blame incompatible Samsung software on a shut-down 3G network for the November 13 failure and warned that “hundreds of thousands” of non‑compliant phones have now been blocked from TPG’s network.

Under questioning from committee chair Sarah Hanson-Young, Berroeta disclosed a second case in which a TPG customer on a Samsung handset could not reach Triple Zero in September. That customer reached emergency services via the National Relay Service about five minutes after the initial attempt.
TPG had already investigated the case as a device-compatibility issue, but was informed by a Telstra staff member on Tuesday that the call “might have been made in connection to someone who has passed away”. The company is seeking confirmation from NSW Ambulance.
